Dr. Darren J. Silvester

DPM, FACFAS, FABFAS, FAENS

Dr. Silvester completed his podiatry training at the California College of Podiatric Medicine in San Francisco. Completing his surgical residency in Corona, California, and since then, has been practicing podiatric medicine for over 25 years.

Dr. Silvester served as president of the South Dakota Podiatric Medical Association and directed the Diabetic Foot Care Program for the Sisseton Indian Health Service for five years. This involved an aggressive patient education program, regular foot screenings, limb salvage intervention including prophylactic surgery, surgical and medical management of infections and ulcerations, as well as organizing a team of internal medicine and vascular surgery physicians to treat the IHS population.

Dr. Silvester is a proud member of several prestigious organizations, including the American College of Foot and Ankle Surgeons, the American Board of Foot and Ankle Surgeons, the American Podiatric Medical Association, the Association of Extremity Nerve Surgeons, the Texas Podiatric Medical Association, and the Academy of Ambulatory Foot and Ankle Surgery. In addition, Dr. Silvester is certified by the Dellon Institutes for Peripheral Nerve Surgery.

To further his reach to those in need, Dr. Silvester opened his flagship Next Step Foot & Ankle Clinic in Universal City, Texas. Now, with two South Texas Foot and Ankle Clinics (Universal city and Pleasanton, TX) and the collaboration with other talented physicians, the opportunities to make a difference in his patients’ lives is boundless. His practice has grown in the San Antonio area to include the most up-to-date and effective methods of dealing with foot and ankle pain.

Dr. Gregory P. Larsen

DPM, AENS

Dr. Larsen completed his podiatry training at Barry University School of Podiatric Medicine in Miami, Florida. He also completed a surgical residency at the Jerry L Pettis Memorial Veterans Hospital in Loma Linda, California. Dr. Larsen is a member of the American Podiatric Medical Association and has been extensively trained in the most up-to-date reconstructive foot and ankle surgery methods and equipment, specifically Arthroscopy and Charcot Foot Reconstruction. He has ample experience in assessing and treating difficult wounds, complex foot deformities, and traumatic injuries.

Due to Dr. Larsen’s love of sports, he is passionate about sports medicine and biomechanics and desires to help improve the function and performance of daily life for all his patients. Dr. Larsen genuinely cares about his patients and finds the most satisfaction in resolving their issues, alleviating their foot and ankle pain, and finding solutions that are tailored to each person. He truly loves working with people and has had patients who wrote him poems and literally gave him the shirt off their back.

Growing up in California, Dr. Larsen feels the most comfortable on a snowboard in the mountains or a skimboard on the beach. However, after meeting his native-to-San-Antonio wife, Kendra, he has come to love Texas BBQ and the friendliness of southern hospitality. Dr. Larsen is fluent in both English and Spanish and will never turn down a carne asada taco. He and Kendra have been married since 2008 and have two sons and two daughters.

Sara E. Grzywa

DPM, AACFAS

Dr. Sara Grzywa, DPM graduated summa cum laude from the University of Texas at San Antonio with a Bachelor of Science in biology. She attended medical school at Des Moines University in Iowa, where she graduated second in her class.

Dr. Grzywa completed her surgical residency at Detroit Medical Center in Michigan, a level one trauma hospital. Her training included all forms of lower extremity pathology, specializing in trauma and reconstructive surgery. Following her residency, she completed one of the most coveted surgical fellowships in the country at the American Foundation of Lower Extremity Surgery and Research in southern New Mexico. There, she received advanced training in complex reconstructive foot, ankle, and leg surgery. Dr. Grzywa is Board Certified in both forefoot and rearfoot and ankle reconstructive surgery. She recently completed the 1st Total Talus surgery in South Texas! Solidifying her prior training and experience. 

She treats all foot and ankle ailments, including bunions, hammertoes, sports injuries, heel pain, sprains, fractures, trauma, and deformities, and can perform total ankle replacements and complex forefoot/rearfoot reconstructive surgery. Dr. Grzywa develops a tailored treatment plan for each patient based on their specific individual needs utilizing the latest technology and procedures. She makes it a priority to educate her patients on their ailments and thoroughly covers all treatment options.

Dr. Sara Grzywa is an energetic, personable, and experienced physician who is committed to providing outstanding medical care. She continues to actively participate in research, surgical workshops, and conferences in order to provide the best care possible for her patients. Dr. Grzywa looks forward to meeting and treating the health care needs of the residents of San Antonio and the surrounding area.

When she is not working, she enjoys spending time outdoors, hiking, biking, and being in the water.
